Selecting a Physician & Making Appointments

  • To schedule an appointment, please call the Hammond Clinic at (219) 836-3600.
  • To become an established patient of the Hammond Clinic, you should select a primary care physician and make an appointment. Your primary care physician can be a family practitioner, general internist, or a pediatrician. If you have not selected a physician, click here to "Find a Doctor" online.
  • Please have your current insurance card in front of you when calling to make an appointment.
  • If you must cancel your appointment, please call as early as possible. You may be billed for a late cancellation if less than 24 hours notice is given prior to your appointment.
  • Your doctor may request a follow-up visit. You will have the best appointment selection if you schedule your next visit at the Reception Desk before leaving the Clinic.
Our Services - How To Use Our Services

Registration

  • Always register for appointments at the Registration Desk on your doctor's floor.
  • Present your appropriate insurance, Medicare, and Medicaid cards when you arrive for an appointment.
  • If your insurance company requires co-payment, you must make payment at the time of registration.
  • If your illness or injury is job-related, you must first obtain written authorization for treatment from your employer. Present that authorization to the receptionist at the Urgent Care Center/Occupational Medicine Department when you arrive to help ensure that your employer is billed.
  • If you are a member of Managed Care, a Hammond Clinic physician must initiate any medical treatment that you receive under the plan.

Emergencies

  • The Hammond Clinic has an Urgent Care facility at 7905 Calumet Avenue, Munster, which is open seven days a week 365 days a year:

    If your primary care physician is unavailable, you may utilize the Urgent Care facility.

  • If you experience an emergency after 8:00PM, please call the Hammond Clinic and speak to the nurse who will refer you to a hospital if necessary. If not, you may be instructed to contact your own physician or Urgent Care for treatment the following day.

Insurance

Referrals

  • For a patient to be referred outside of the Hammond Clinic, the patient's primary care physician must first determine that Hammond Clinic cannot provide such care.

If a patient has an HMO provider, the patient's primary care physician will submit a referral request to the Managed Care Department. If the referral is approved, the patient may pick up the referral at the Managed Care Department, or it will be mailed to the patient. If the referral is denied, an alternate plan or treatment will be recommended; the patient will be advised of the reason for denial.
